Refer a Patient

If you wish to refer a patient for Physical or Occupational Therapy just give us a call or fax us a demographic face sheet and a prescription. We will take care of the rest.

We will keep you updated with timely Initial evaluations and monthly progress reports. We are always available to discuss any details and requests regarding your patients.

As long as there is NOT a current Medicare Part A case open, we are able to provide our services to patients who have Medicare and it is covered by Medicare Part B and secondary insurances.
