Back to Health’s “Therapy at HOME “ program provides Physical and Occupational Therapy services to geriatric, post-operative orthopedic and neurologically impaired patients. Our physical and occupational therapists can provide therapy in the comfort of your own home. Our team will bring the clinic to you!

Our highly skilled, tenured therapists provide outstanding, one-on-one care directly to your home – with the same treating therapist every session.

We bring any necessary equipment that will help you achieve all your goals. Equipment may include but is not limited to portable treatment tables, ultrasound, electric stimulation, weights, portable bicycles hot packs, and therabands, to name a few.

Physical Therapy treatment plans can focus on:

  • regaining strength
  • improve walking
  • increase balance
  • fall prevention
  • pain reduction
  • easier coordination
  • mobility

Occupational therapy treatment plans can focus on:

  • “activities of daily living”, such as cooking, bathing, and dressing
  • fall prevention
  • transferring safely, among other benefits to both the patients and the caregivers.

We specialize in treating a wide array of disorders including but not limited to:

  • Difficulty in walking, getting out of a chair or bed
  • Difficulty in bathing, dressing, grooming or other activities of daily living
  • Balance problems or fear of falling
  • Stiff and painful joints
  • Total hip and knee replacements, in addition to other orthopedic surgeries.
  • Neurological conditions such as Parkinsons, Stroke, Multiple Sclerosis and Neuropathies
  • Decreased strength and endurance
  • Back and neck pain
  • Knee and shoulder pain
  • Vision or cognitive issues
  • Dependency on a caregiver

Back To Health Physical Therapy’s “Therapy at Home” services are covered by Medicare Part B – providing Outpatient Physical and Occupational Therapy in the home, instead of in a clinic. We also work with many other insurance companies, secondary insurances, and private pay.

We are not a traditional “Homecare Agency” – covered by Medicare Part A, which is applicable only for home-bound status patients.

Many of our patients receive our “Therapy at Home” program after they have completed their Part A, homecare agency, therapy program and are ready to begin outpatient therapy. To clarify the difference between Medicare Part A and Medicare Part B:

Will my insurance company pay for THERAPY AT HOME if I am not homebound?

Back To Health’s in-home therapy is covered under Medicare Part B and most other insurances. Call our offices to have our team verify your insurance.

HOW DO YOU GET STARTED WITH BACK TO HEALTH?

Call our team and we will do the rest!

All we need is to do a brief intake with you by phone, and a prescription for physical or occupational therapy from your doctor – you can ask your doctor for a prescription, or we can do that for you.

Who will come to my home?

Back To Health Physical Therapy has assembled a team of outstanding, caring, skilled and tenured physical and occupational therapists. Our Back to Health team truly cares about their patients, and each member of the team strives to return each of our patients BACK to HEALTH!

Once we have your prescription and have completed our initial intake, a tenured, licensed physical or occupational therapist will contact you to arrange dates and times to come to your home.

Our team of therapists have experience in Geriatrics, Neurology, Orthopedics, Cardio-Pulmonary, Vestibular, Wheelchair Seating and Mobility, Durable Medical Equipment and Home Modifications, among other specialties. We never hire new graduates to ensure that you have an experienced therapist who can work independently in your home.
